 What is Affordable Choice?


Affordable Choice is a fixed indemnity insurance plan underwritten by ManhattanLife, offering guaranteed daily benefits for hospital stays, surgeries, and routine medical services, regardless of provider charges. With no deductibles, 1st-dollar coverage and a range of plans from Elite Plus to Classic, this plan provides customizable coverage for individuals, families, and businesses in Hawai'i. --- 4-minute overview video below ---


Who should consider this new health insurance plan?


This plan is ideal for anyone who meets the following criteria:


  • Anyone who does not qualify for MedQuest or tax-subsidies for ACA-plans on healthcare.gov because their income is too high.
  • Anyone who wants to save 30%-80% on ACA-plans (which could = $1,000+ in premium savings each month)
  • Anyone who is healthy and can pass the 8 underwriting questions.
  • Anyone not getting pregnant (excluded on the policy).

The Affordable Choice base plan provides daily cash benefits for routine and hospital care. The optional Specified Disease Rider (SDR) takes it a step further — providing up to $500,000 in calendar year benefits specifically for major, life-altering diagnoses like cancer, heart attack, stroke, and organ transplants.


Think of it as your financial backstop for the big stuff.


How It Works: The SDR pays the difference between your actual medical bills and what the base plan covered — after a deductible you choose ($25K, $50K, $75K, or $100K). Your calendar year maximum is either $250K or $500K.

Affordable Choice Enhanced is a fixed-indemnity hospital insurance plan that pays set cash amounts for covered services, like doctor visits, hospital stays, surgery, cancer treatments, prescriptions, and preventive care, with no deductibles or coinsurance. 


When you receive covered care, the plan pays the flat dollar amounts listed in the benefit schedule, regardless of what the provider charges; you can assign benefits to the provider (recommended - have the provider file the claim) or have the benefits paid to you to help with medical bills or other expenses. 


No. This is not major medical coverage and it does not meet ACA “Minimum Essential Coverage” requirements; it’s designed to provide valuable first-dollar cash benefits and work alongside (or instead of) traditional health insurance, depending on your situation. 


This plan tends to work well for reasonably healthy individuals, families, and small-business owners who want strong benefits for everyday care and unexpected accidents or illnesses, but need a more affordable option than many ACA bronze plans—especially when combined with other coverage like accident or critical illness insurance. 


You can apply for Affordable Choice Enhanced any time during the year. There is no limited “open enrollment” window for this plan. 


Yes, Affordable Choice Enhanced is an underwritten product, which means there are a handful of simple medical questions on the application and coverage is not guaranteed-issue; most generally healthy people are able to qualify. 


The Hawaiʻi Enhanced version pays fixed cash benefits for inpatient hospital stays, hospital admission, surgery (inpatient and outpatient), emergency room and urgent care visits, doctor office visits with rollover, preventive services, lab work, imaging (like X-rays, MRIs, CTs), ground and air ambulance, cancer treatments (radiation, chemo, immunotherapy), prescriptions, and certain therapy services (including chiropractic), up to stated calendar-year maximums. 


Yes. There’s no requirement to change doctors; you can see any licensed provider, but you’ll generally save the most by using providers in the First Health LBP Network, which offers discounted rates at hundreds of thousands of physicians, hospitals, and ancillary facilities nationwide. 


There are no deductibles or coinsurance on Affordable Choice Enhanced. Benefits start from the first covered service and are paid at the fixed amounts in the schedule, although certain services and conditions are subject to plan limitations and exclusions outlined in the policy. 


Yes; many clients pair Affordable Choice Enhanced with an ACA marketplace plan, employer coverage or other ManhattanLife products (like accident, critical illness, or hospital indemnity/GAP) to create a more affordable “stacked” package that boosts their protection while keeping premiums manageable.
